INFORMATION ABOUT THIS STUDY:

Why are you being asked to be in this study?
Tapestry wants to understand how its students form friendships, and whether friendships affect your school experiences, mental health, or academics. You are being asked to participate since you are currently a student at Tapestry Public Charter School.

What will happen to you if you take part in the study?
If you agree to take part in this study, you will be asked to fill out a series of questionnaires. We will ask about your friends, your feelings about school, and your moods. This will take about one hour.

Will any part of the study make me feel uncomfortable?
Some of the questionnaires ask about your friends, and others ask about how often you feel sad. If these questions make you uncomfortable, you can stop participating at any time, or you may speak to a guidance counselor. There is no penalty for stopping.

Can you change your mind about being in the study at any time?
Yes. You can change your mind about being in the study at any time. There is no penalty for changing your mind.

Do you have to be in this study even if you don't want to?
You do not have to be in this study if you do not want to. You should only sign below if you want to be in this study.
