Role of the Airway Epithelium in Children with Asthma

Asthma is one of the most common long-term illnesses among children. Illness related to childhood asthma lead to more than 640,000 emergency department visits, 280,000 hospitalizations, and 13.8 million missed school days per year in the U.S. Studies that follow children with asthma into their teenage years and adulthood show that at birth, lung function is normal in children who develop asthma, but by age six, it can decrease due to airway injury.

One goal of this study is to compare between children with and without asthma ways that the cells lining the airways (epithelial cells) in your lungs signal other cells so that we may learn why some children with asthma experience decreases in lung function. We want to compare this in children with asthma and children without asthma or allergies. We are looking for 250 children in each group who are scheduled to undergo surgery that requires anesthesia to take part. In our research laboratory we are completing experiments that compare the function of cells from the nose and windpipe between children in these two groups.

Respiratory virus infections are the primary trigger of up to 90% of asthma attacks. There are standard medications to treat asthma that are effective at improving symptoms and reducing the frequency of attacks, yet there are still millions of children in the U.S. who experience severe asthma attacks triggered by viral infections every year. Another goal of this study is to understand how the cells lining your airways affect the frequency of asthma attacks in asthmatic children. We want to look at specific genes in your airway cells that play a role in the protection against respiratory viruses and to try to understand if there is a connection between the function of these genes and the risk and frequency of asthma attacks triggered by viruses.

If your child were to participate in this study, while they are asleep from anesthesia for surgery a study doctor would quickly brush the inside of their nostrils and windpipe to gather some cells. Your child would also have a small amount of blood drawn (one teaspoon) while asleep to test for allergies to dogs, cats, dust mites, grass and mold. For asthma participants, we would also collect an extra amount of blood (1/4 teaspoon) to test for eosinophil count (a marker for immune system inflammation). We would also ask you questions about your child's medical history.

A follow-up study visit would be scheduled several weeks or months later after your child has recovered from his/her surgery. During the study visit, your child would be asked to breathe into two different machines, and you would be given the results of the allergy testing. This would take about 30 minutes to complete.

You would not be charged for any of the tests. Your child would receive a re-loadable $25 debit card at the follow-up study visit for their time and participation.
