Introduction

Penn Medicine is currently engaged in the FY2022 federally mandated Community Health Needs Assessment (CHNA) process, which will inform our Community Health Implementation Plan (CHIP). In order to maximize impact with our community health initiatives, it is necessary to first understand the current state of activity. We know there is a wealth of great work going on across Penn Medicine and the Perelman School of Medicine, and we want to make sure we capture all of your important work. At this time, we aim to collect data on all community health initiatives at Clinical Care Associates (CCA), Clinical Practices of the University of Pennsylvania (CPUP), Hospital of the University of Pennsylvania (HUP), Pennsylvania Hospital (PAH), Penn Presbyterian Medical Center (PPMC), and the Perelman School of Medicine. We plan to expand these efforts in the future to include the entire health system.

Community health initiatives encompass strategic approaches for improving health, preventing disease, and reducing health disparities by addressing social, behavioral, environmental, economic, and medical social determinants of health in a geographically defined population (American Hospital Association, 2017). Examples of community health initiatives include, but are not limited to: free breast cancer screenings, tobacco cessation programs, life skills workshops, community flu clinics, nutrition programming, and exercise workshops. The following activities are not considered to be community health initiatives, thus are not intended to be captured by this survey: clinical activities within the Penn Medicine health system, individual volunteer efforts among Penn employees, or activities that target Penn Medicine employees.

There are 5 main parts of this survey: general program information; brief funding and budgetary information; evaluation and reporting methods; Penn and community involvement; and program contact information. If you have your program information on hand, you should be able to complete this survey in 5-10 minutes.
