The Statement of Financial Affairs is included in every Chapter 7 and Chapter 13 bankruptcy petition. It is also known as Official Form 7.
The form requires your attorney to list a variety of information, including your income for the last three years, payments to creditors for the past 90 days which exceed $600.00, any lawsuits you are involved in, repossessions or foreclosures, and so forth.
When you review your bankruptcy petition with your attorney, pay careful attention to the Statement of Financial Affairs. If the trustee sees anything unusual, you can expect questions at the meeting of creditors.
You can find a copy of the form here.
