STUDY TITLE: Perspective on Healthcare Access & Wellbeing for Individuals with and without Chronic Lung Disease During the COVID19 Pandemic

Why is this study being done?
The purpose of the study is to determine how, the wellbeing and access to healthcare, of individuals with chronic lung disease and chronic medical conditions has been affected by the COVID19 pandemic. You are being asked to participate because we are interested in knowing your perspective and hope to make improvements in future healthcare based on your response.
What will happen if I take part in this study?
It will take you approximately ten minutes to complete this survey online. You will be asked questions about your emotional and physical wellbeing as well as healthcare access before and during the COVID19 pandemic.
What are the risks of taking part in this study?
As this is an anonymous survey we will not know who participated or be able to trace your individual responses. We believe that this survey study presents no risks greater than those experienced in everyday life.
Are there any benefits to participating in this study?
There will be no direct benefit to you from participating in this study. However, your voice will be heard and will provide important information for improving care and healthcare access.
