After the implementation of our Inclusion, Cultural Humility, Diversity and Equity (INCLUDE) curriculum, our residency would like to better assess its effectiveness and impact on your clinical practice.

The questions below are based upon the INCLUDE needs assessment that was previously administered in 2018 as well as use of published tools (1,2) that evaluate cultural competency for health professionals. It has undergone revisions with input from Diversity Committee, Residency Committee and GME.

Please complete this assessment honestly and openly. Your participation is voluntary and your responses are confidential. Residents who completed the initial INCLUDE needs assessment in 2018 were told that their responses were anonymous; however, the REDCap system retains a link to the respondent’s email address. The information is still kept confidential and the investigators do not access, use, or analyze responses linked to individuals. Residents who complete this post-implementation survey may have their responses linked for pre- and post- analysis, but we will never use individual identifiers or identify individual participants.

We intend to analyze and share the results of this survey within our residency as well as for the purposes of research. All data will be shared in the aggregate without individual identifiers.
